What is Compounded Tirzepatide?

Tirzepatide is a dual-action medication — it works on two of your body's appetite hormones, GLP-1 and GIP, while most GLP-1 treatments target just one. Together they help curb appetite, slow digestion, and keep you feeling full longer. In head-to-head clinical trials, tirzepatide produced greater average weight loss than semaglutide.

Our compounded version is prepared by a licensed US compounding pharmacy and can be personalized to your needs — sometimes formulated with additions like B vitamins or glycine to help with tolerability.

Compounded medications are permitted to be prescribed under federal law, but they are not FDA-approved and do not undergo FDA review for safety, effectiveness, or manufacturing quality. Your provider will determine whether compounded tirzepatide is clinically appropriate for you.

How to take

Compounded tirzepatide is a once-weekly injection you give yourself at home, just under the skin (subcutaneously) — typically in the abdomen, thigh, or upper arm. Take it on the same day each week, at any time of day, with or without food, and rotate your injection site each time.

Your provider sets your starting dose and a titration schedule that gradually increases it as your body adjusts — this is key to managing side effects. You'll receive clear, step-by-step instructions with your medication, and your care team is available for any questions. Always follow your provider's specific directions

Potential side effects

Like all medications, tirzepatide can cause side effects. The most common are gastrointestinal and tend to ease as your body adjusts: nausea, diarrhea, vomiting, constipation, abdominal discomfort, and decreased appetite. Some people also notice fatigue, headache, or mild injection-site reactions.

Less common but serious effects can include pancreatitis, gallbladder problems, kidney issues, low blood sugar (especially if combined with other glucose-lowering medications), and allergic reactions. Tirzepatide is not recommended for people with a personal or family history of medullary thyroid cancer or Multiple Endocrine Neoplasia syndrome type 2 (MEN-2).

This is not a complete list of side effects. Your provider will review the full risk profile with you, and your care team is available throughout treatment. Contact your provider or seek medical care right away if you experience severe or persistent symptoms. See our Important Safety Information for full details.

A DEEPER LOOK INTO TIRZEPATIDE

Meet the Double Agent

Remember food noise, that constant mental chatterabout what to eat next? Semaglutide turns the volume down with one hormone.Tirzepatide reaches for two.

Two pathways. Twice the teamwork.

Your body makes two appetite hormones that matter here: GLP-1 and GIP. GLP-1 helps you feel full and slows digestion. GIP works alongside it, shaping how your body handles hunger and energy. Most treatments target just one of them.

Tirzepatide is built to activate both. That's what "dual-action" actually means one medication working on two of your body's hunger pathways at once. In head-to-head studies, that two-pronged approach led to greater average weight loss than a GLP-1 alone.¹

What makes it different

Two hunger pathways targeted, not one

Cravings and "food noise" turned down even further

Feeling full sooner, and staying full longer

Greater average weight loss than semaglutide in head-to-head trials¹ ²

Common questions about Compounded Tirzepatide

Visit our FAQ page for more

What makes tirzepatide different from semaglutide?

Tirzepatide is dual-action — it works on two appetite pathways (GLP-1 and GIP), while semaglutide works on one. Your US board-certified physician helps you choose the right fit for your body and goals.

Is compounded tirzepatide FDA-approved?

No. The active ingredient, tirzepatide, has been FDA-approved, but compounded medications themselves are not FDA-approved for safety, efficacy, or quality. Yours is prepared by a US-licensed pharmacy and prescribed only after a licensed clinician reviews your health history.

How much weight could I actually lose?

Your results depend on your body, your dose, and your habits — which is exactly why a physician guides the whole process.

Is it safe, and what about side effects?

Like any medication, tirzepatide can have side effects — most commonly mild GI ones that often ease as your body adjusts. That's why CMWL starts you low, titrates carefully, and monitors you throughout. The addition of glycine also helps to reduce GI side effects like nausea. Your care team is there to manage anything that comes up.

Who decides my dose, and does it change over time?

A US board-certified physician sets your starting dose and adjusts it as your body responds — a process called titration. You're never left to figure it out alone. And if you’re already actively on tirzepatide and switching to CMWL, you won’t have to start over.

What's included in the price?

One all-in price covers your medication, supplies, and clinical care — no separate membership fee and no hidden costs. You also get FREE access to CMWL Reset lifestyle and behavior education.

How soon can I start, and when will my medication arrive?

Intake takes about 5 minutes, and you'll typically hear from a clinician within 24–48 hours. If you're approved, your medication ships FREE from a US-licensed pharmacy straight to your door in 3-5 days.

What if it's not right for me — can I pause or cancel?

Yes — pause or cancel anytime, no lock-in or penalty. And if a clinician determines treatment isn't right for you, you're not charged for it.

Why choose CMWL over an app-based provider?

Because CMWL isn't an app with a prescription button — it's the largest doctor-led weight-loss network in the U.S., with 1000+ CMWL-trained medical providers over its 20+ year history as a leader in medical weight loss. You get a real physician and ongoing care, not an algorithm.

Which states is this available in?

CMWL is available in all 50 US States. Availability can change, so your eligibility is confirmed during intake.
