Compounded semaglutide is a once-weekly injection you give yourself at home, just under the skin (subcutaneously) — typically in the abdomen, thigh, or upper arm. Take it on the same day each week, at any time of day, with or without food, and rotate your injection site each time.

Your provider sets your starting dose and a titration schedule that gradually increases it as your body adjusts — this is key to managing side effects. You'll receive clear, step-by-step instructions with your medication, and your care team is available for any questions. Always follow your provider's specific directions.

Potential side effects

Like all medications, semaglutide can cause side effects. The most common are gastrointestinal and tend to ease as your body adjusts: nausea, vomiting, diarrhea, constipation, and abdominal discomfort. Some people also notice fatigue, headache, or mild injection-site reactions.

Less common but serious effects can include pancreatitis, gallbladder problems, kidney issues, and allergic reactions. Semaglutide is not recommended for people with a personal or family history of medullary thyroid cancer or Multiple Endocrine Neoplasia syndrome type 2 (MEN-2).

This is not a complete list of side effects. Your provider will review the full risk profile with you, and your care team is available throughout treatment. Contact your provider or seek medical care right away if you experience severe or persistent symptoms. See our Important Safety Information for full details.

Treatment is provided as part of a medical weight loss care program. Enrollment includes a virtualconsultation with a licensed provider, who will determine whether treatment is clinically appropriate.Medication is prescribed only if approved by the provider. Program pricing reflects medical evaluation, provider oversight, and ongoing care. Medication type, dosage, and final treatment plan may vary based onclinical assessment. I am very happy this is working, and it is teaching me to not overeat, and not to eat out of boredom.

I’m going to try the education program.  Generally, I know what I should do but have some weird mental resistance.  I need long term success due to heart disease that runs in my family.

A DEEPER LOOK INTO SEMAGLUTIDE

Meet the Hormone That Turns Down the Noise

Food can be loud. The 3 p.m. snack that
won't stop calling your name. The "just one more" that becomes five. The little tug-of-war you fight a dozen times a day.
There's a name for it, food noise and it's exhausting.

Your body already has the volume knob

There's a hormone built to turn that noise down. It's called GLP-1. After you eat, it helps you feel full, slows things down, and tells your brain "we're good." The problem? For a lot of us, it just doesn't speak up loudly enough.¹ ²

That's where semaglutide comes in. It mimics your natural GLP-1 — so fullness lasts longer, cravings get quieter, and food stops running the show.

What people notice

Eating until satisfied, not stuffed

Strolling past the snack drawer without a second thought

Way fewer "why am I still thinking about lunch?" moments

Feeling like you're finally in the driver's seat³

Results may vary.

Common questions about Compounded Semaglutide

Visit our FAQ page for more

Why might semaglutide be a good starting point if I’m new to GLP-1 medication?

It's the original most popular GLP-1 medication, it's our most affordable plan, and it has a strong track for weight loss. For many people, it's the simplest proven way to begin. Your US board-certified physician confirms whether it's the right starting point for you.

Is compounded semaglutide FDA-approved?

No. The active ingredient, semaglutide, has been FDA-approved, but compounded medications themselves are not FDA-approved for safety, efficacy, or quality. Yours is prepared by a US-licensed pharmacy and prescribed only after a licensed clinician reviews your health history.

How much weight could I actually lose?

It varies. Your results depend on your body, your dose, and your habits — which is why a physician guides the process.

Is it safe, and what about side effects?

Like any medication, semaglutide can have side effects — most commonly mild GI ones that often ease as your body adjusts. CMWL starts you low, titrates carefully, and monitors you throughout, with a care team ready to help. The addition of B Vitamins or glycine also help minimize GI side effects like nausea.

Who decides my dose, and does it change over time?

A US board-certified physician sets your starting dose and adjusts it as your body responds — a process called titration. You're never left to figure it out alone. And if you’re already actively on semaglutide and switching to CMWL, you won’t have to start over.

What's included in the price?

One all-in price covers your medication, supplies, and clinical care — no separate membership fee and no hidden costs. You also get FREE access to CMWL Reset lifestyle and behavior education.

How soon can I start, and when will my medication arrive?

Intake takes about 5 minutes, and you'll typically hear from a clinician within 24–48 hours. If you're approved, your medication ships FREE from a US-licensed pharmacy straight to your door in 3-5 days.

Can I switch to tirzepatide later?

Often, yes. Many people start on semaglutide and move to tirzepatide if their physician recommends it — because you're with a real care team, your plan can evolve with you.

What if it's not right for me — can I pause or cancel?

Yes — pause or cancel anytime, no lock-in or penalty. And if a clinician determines treatment isn't right for you, you're not charged for it.

Why choose CMWL over an app-based provider?

Because CMWL isn't an app with a prescription button — it's the largest doctor-led weight-loss network in the U.S., with 1000+ CMWL-trained medical providers over its 20+ year history as a leader in medical weight loss. You get a real physician and ongoing care, not an algorithm.

Which states is this available in?

CMWL is available in all 50 US States. Availability can change, so your eligibility is confirmed during intake.
